Model Exploration and Evaluation: Investigate and assess various MLTC models currently in use within the NHS and other healthcare systems. Identify best practice and strategies that can be effectively implemented across Bromley.
Implementation Support: Collaborate with GP practices to support the design and roll out of MLTC interventions, helping practices to integrate new processes and overcome any challenges encountered.
Training and Education: Work with the ICB and the Bromley Training Hub to develop and deliver training sessions for practice staff. Help practices to ensure that all team members understand their roles within MLTC care.
Monitoring and Feedback: Support analysis and audit assessing the impact of MLTC innovation on patient outcomes. Work with practices to review feedback from patients and practice staff to continuously improve the new model of care.
Good Practice Sharing: Create and disseminate resources, case studies, and guides that highlight best practice for MLTC. Facilitate forums, workshops, or webinars where practices can share experiences and learn from one another.
Stakeholder Engagement: Engage with key stakeholders, including primary care networks (PCNs), commissioners, and patient groups, to align MLTC initiatives with broader healthcare goals and ensure they meet the needs of the local population.
Research and Development: Stay updated on the latest research, guidelines, and innovations in MLTC. Contribute to the body of knowledge by sharing insights and experiences with the wider healthcare community.

Reporting and Documentation: Support recording of the implementation process, outcomes, and lessons learned. Provide regular reports to the Bromley Training Hub and other relevant bodies, highlighting successes, challenges, and recommendations for future improvements.
